Step 7: Complete your Immunization Record
We care a lot about your academic and social life and we also care about your personal health and well-being. At UMD it is required that all students have an Immunization Record on file with the University Health Center. Please look in the booklet you received called "Get to Know Your Maryland: Spring 2008 Guide to Orientation." Please note that the line marked "UID" is not your social security number. Please leave it blank for now.
You can find the Immunization Record Form by clicking here.
The University Health Center is a full service health facility in the center of campus across the street from the Stamp Student Union. All students who pay their fees may use this facility by calling the appointment line at 301-314-8184. All patients must call this number to set up all appointments. This is the only way appointments are scheduled. However, urgent care emergencies are available. Patients with emergencies must check in at the front desk of the Health Center.
There are many services available to students, faculty and staff. Some of these include:
Acupuncture
Allergy Clinic
Dental Health Clinic
Eating Disorder Treatment
Emergency Contraception
Immunizations
International Travel Clinic
Laboratory
LGBT Student Concerns
Massage Therapy
Occupational Health
Pharmacy
Primary Care

And more…
We highly recommend:
-Providing the pharmacy with your pharmacy insurance information
-Visiting the Health Center website atwww.health.umd.eduto find a complete listing of services offered
-Finding out about the University Health Insurance Plans for students and finding out if your insurance covers services beyond regular check-ups.
You should also know…
-Each visit to the Health Center costs $10 for students (except some specialist), additional charges may apply for x-ray, lab, pharmacy, etc.
-The only health insurance accepted for medical visits is UnitedHealthCare.
-The Pharmacy will accept most insurance plans.
-All fees will be assessed to your student account. You may also pay by cash, check or credit.
